   Please complete this form to obtain an Annapolis Fire Department FIRE Report. Once we have determined that we were the department that handled your call for assistance, you will be given direction on when your report will be ready for pick up at the Fire Department Headquarters or be mailed or emailed. You will also be advised of any applicable confidentiality requirements. Please be advised that due to ongoing investigations, some reports may not be immediately ready for release.

   To obtain an Annapolis Fire Department EMS Report please contact the City of Annapolis Fire Department Headquarters at 410-263-7975 ext. 0 between the hours of 8:30 a.m. - 4:30 p.m. during normal business days.

   NOTE:  Please note responses may take up to 15 business days.  The Fire report and or any questions will be sent to the email address entered below.
