Camper Health History - Also, an immunization record obtained from the child's physician must be submitted one month from the date of registration. This applies to all camps. If this form is not turned in within the one month timeframe, your child will be withdrawn from the camp(s). You will then risk losing your child’s space in the camp(s).

Maryland Immunization Form - to be completed and signed by child's physician

Permission for Transporation / Swimming / Sunscreen - please complete all three sections

Authorized Release Form for Extended Day

Rock Climbing Wall Release & Waiver - only needed for the Rock Wall Splash Camp (July 23-27 and August 6-10)
